To evaluate the effect of uterine artery embolization (UAE) for symptomatic leiomyomas in patients with uterine sarcomas. Thirty-six consecutive patients with symptomatic uterine sarcomas who had not undergone hysterectomy were treated with UAE. All patients had undergone UAE for symptomatic leiomyomas previously, and 23 patients had had at least one prior hysterectomy. Twenty-eight women had grade 1, 1 had grade 2, and 4 had grade 3 uterine sarcomas. Twenty-eight of the 36 patients had undergone UAE for symptomatic leiomyomas, and there were no significant differences between the uterine sarcoma group and the group of patients who had undergone UAE for symptomatic leiomyomas (P >.1). The mean time to follow-up was 38.7 months. There were no significant differences between the leiomyoma group and the uterine sarcoma group in terms of the success of uterine preservation (93% [28 of 31]) versus 96% [26 of 28]; pelvic side-wall preservation (84% [25 of 29] versus 80% [24 of 30]; and hematologic spread (5% [2 of 37] versus 16% [5 of 31]). Twenty-nine of 30 (97%) patients with leiomyomas and 8 of 10 (80%) with uterine sarcomas were cured by UAE. One patient with uterine sarcoma had recurrence of her primary disease. There were no complications attributable to UAE. Uterine artery embolization is a safe and effective alternative to hysterectomy for the treatment of patients with uterine sarcomas.
